The pre-assessment helps our pastors understand your needs and prayer requests before your session. It ensures that your time together is focused, confidential, and guided by wisdom and discernment.

The form includes simple questions about your personal background, reason for seeking pastoral care, and any previous steps you’ve taken (such as counselling, prayer, or ministry involvement). All information is kept private and secure.

Most people complete the pre-assessment in about 5–10 minutes. Take your time to answer honestly — your responses help us provide the right support.

Once you submit your form, a member of our pastoral team will review it and contact you to schedule your session. You’ll receive confirmation by email or phone with your appointment details.

Yes. All information shared through the pre-assessment and during your session is treated with the highest confidentiality and pastoral sensitivity. Only authorized ministry staff will have access to your details.

Yes, you may indicate a preference on the form. We’ll do our best to accommodate your request depending on availability and ministry areas.
